Title: Protocolo de ejercicios combinando los programas NEMEX y fortalecimiento muscular específico en pacientes con osteoartrosis de rodilla en fase preoperatoria. Ensayo clínico aleatorizado

Abstract: Introducción: La osteoartrosis de rodilla (OA) es un proceso degenerativo articular como consecuencia de diferentes factores. La principal sintomatología de esta patología son el dolor y la incapacidad funcional. Para poder diagnosticarla se utiliza la radiología y la exploración física. El tratamiento puede tanto conservador mediante rehabilitación y/o farmacología como por intervención quirúrgica. Se plantea la combinación de dos métodos de tratamiento para esta patología durante la fase preoperatoria. Objetivo: Evaluar la efectividad que tiene combinar el programa NEMEX con un fortalecimiento muscular específico sobre funcionalidad del paciente, el dolor, la fuerza en miembros inferiores y la kinesiofobia. Metodología: La muestra pertenece al servicio de traumatología del Hospital Royo Villanova de Zaragoza pendientes de una artroplastia de rodilla. La muestra se distribuye de manera aleatoria en tres grupos: Grupo fortalecimiento (GF) únicamente hace ejercicios de fortalecimiento muscular; Grupo experimental (GNF) realizan tanto el protocolo NEMEX como el fortalecimiento muscular específico; Grupo control (GC) recibe sesiones de educación postural e información de la patología. El estudio es durante 6 semanas. Se evalúa la funcionalidad mediante la encuesta Knee injury and Osteoarthritis Outcome Score (KOOS) y el Cuestionario WOMAC; fuerza de miembros inferiores con 30s Chair Stand Test (30s-CST); dolor con la encuesta KOOS, cuestionario WOMAC y la Escala Visual Analógica (EVA); kinesiofobia con Tampa Scale for Kinesiophobia (cuestionario TSK-11SV). Se hacen tres evaluaciones, una al inicio del estudio (semana 0), al finalizarlo (semana 6) y una última 3 semanas después del estudio. Resultados esperados: Mejorar la funcionalidad del paciente y reducir los problemas que produce la operación como la atrofia muscular, el dolor y la kinesiofobia que genera al paciente. Conclusión: La combinación de ambos métodos de tratamiento mejoran la funcionalidad del paciente y reducen los inconvenientes generados tras la intervención quirúrgica.
Description: Introduction: Knee osteoarthrosis (OA) is a degenerative joint process resulting from different factors. The main symptoms of this pathology are pain and functional disability. In order to diagnose it, radiology and physical examination are used. The treatment can be conservative through rehabilitation and/or pharmacology as well as surgical intervention. A combination of two treatment methods is proposed for this pathology during the preoperative phase Objective: To assess the effectiveness of combining the NEMEX programme with specific muscle strengthening on patient function, pain, lower limb strength and kinesiophobia. Methodology: The sample belongs to the traumatology service of the Royo Villanova Hospital in Zaragoza pending a knee arthroplasty. The sample is randomly distributed into three groups: Strengthening Group (GF) only does muscle strengthening exercises; Experimental Group (GNF) performs both the NEMEX protocol and specific muscle strengthening; Control Group (GC) receives postural education sessions and pathology information. The study lasts for 6 weeks. Functionality is evaluated using the Knee injury and Osteoarthritis Outcome Score (KOOS) survey and the WOMAC Questionnaire; lower limb strength with 30s Chair Stand Test (30s-CST); pain with the KOOS survey, WOMAC questionnaire and the Visual Analogue Scale (VAS); kinesiophobia with Tampa Scale for Kinesiophobia (TSK-11SV questionnaire). Three evaluations are done, one at the beginning of the study (week 0), at the end of the study (week 6) and one last 3 weeks after the study. Expected results: Improve the functionality of the patient and reduce the problems produced by the operation such as muscular atrophy, pain and kinesiophobia that generates the patient. Conclusion: The combination of both treatment methods improves the patient's functionality and reduces the inconvenience generated after surgery.
