Título : Efectividad del entrenamiento de marcha asistido por robot de un paciente tetrapléjico: a propósito de un caso

Resumen : Introducción y objetivo: Las lesiones de la médula espinal interrumpen las vías ascendentes y descendentes que transportan la información entre el cerebro y la médula espinal, lo que provoca graves deficiencias en las funciones motoras, sensoriales y autónomas en el lugar de la lesión y debajo de esta. El objetivo del presente estudio es valorar la efectividad del entrenamiento de marcha asistido por robot en un paciente tetrapléjico. Metodología: Se realizó un tratamiento fisioterapéutico durante 5 semanas, compuesto esencialmente del Lokomat con realidad virtual, pero también de estiramientos, fortalecimiento muscular y verticalización. Se realizaron valoraciones al inicio y al final del tratamiento. Se evaluaba la velocidad de marcha mediante la prueba de 10-meter walk test; la distancia de marcha mediante la prueba de 6-minute walk test; el grado de deterioro en relación con la función de marcha mediante las adaptaciones necesarias con la prueba de Walking Index for Spinal Cord Injury II; la fuerza muscular con la escala Lower Extremity Motor Score; la independencia funcional con la escala The Spinal Cord Independence Measure y la espasticidad con la escala de Ashworth Modificada. Resultados: Los resultados muestran una mejoría en todas las variables, sobretodo en la capacidad funcional de la marcha (mejoría tanto en la velocidad como la distancia de marcha) y en la fuerza muscular. Conclusiones: El estudio demostró que el uso del entrenamiento de la marcha asistido por robot con realidad virtual aportó beneficios a nivel de marcha, pero también a nivel de fuerza muscular, espasticidad e independencia funcional en un paciente tetrapléjico.
Descripción : Introduction and objective: Spinal cord injuries disrupt the ascending and descending pathways that carry information between the brain and spinal cord, resulting in severe deficiencies in motor, sensory, and autonomic functions at and below the site of injury. The aim of this study is to assess the effectiveness of robot-assisted gait training of a quadriplegic patient. Methods: A physiotherapeutic treatment was carried out for 5 weeks, consisting essentially of Lokomat with virtual reality, but also of stretching, muscle strengthening and verticalization. Evaluations were carried out initially and finally at the treatment, as well as 5 weeks after the end of the treatment. The walking speed was evaluated with the 10-meter walk test; the walking distance with the 6-minute walk test; the degree of deterioration in relation to the walking function through the necessary adaptations with the Walking Index for Spinal Cord Injury II test; the muscle strength with the Lower Extremity Motor Score; the functional independence with the Spinal Cord Independence Measure scale and the spasticity with the Ashworth Modified scale. Results: The results show an improvement in all variables, especially in the functional capacity of the gait (improvement in speed such as walking distance) and muscle strength. Conclusions: The study demonstrated that the use of robot-assisted gait training with virtual reality provided benefits at the gait level, but also for muscle strength, spasticity, and functional independence of the patient in a quadriplegic patient.
