Título : Coste-efectividad de técnicas de fisioterapia invasiva

Resumen : Introducción: La punción seca es una técnica mínimamente invasiva que permite tratar diferentes afecciones del sistema neuromusculoesquelético directa (ej. contracturas musculares, lesiones tendinosas) e indirectamente (ej. dolor crónico y espasticidad). Para ello se utiliza una aguja sólida, no biselada, que provoca un efecto mecánico sobre el tejido y puede combinarse con otros agentes físicos como las corrientes eléctricas, en función de los objetivos del tratamiento. Varios estudios han evaluado ya su efectividad en diversas patologías, sin embargo, su impacto económico todavía no se ha medido y su posible incorporación a la práctica clínica diaria en casos como en la rehabilitación de pacientes con ictus puede suponer un gran ahorro en costes comparado con las alternativas actuales. El objetivo de la presente tesis es determinar cuál es el posible coste de estas técnicas en diferentes aplicaciones y realizar el análisis de coste-efectividad para valorar su eficiencia en cada una de ellas. Material y métodos: Se han completado cuatro estudios de coste-efectividad de estas técnicas mínimamente invasivas en diferentes ámbitos: 1) El primer estudio es un análisis de coste-efectividad que compara la técnica de punción seca con la electrólisis percutánea en el dolor plantar y de talón. El objetivo fue determinar el coste de ambas técnicas y a partir de los datos de un ensayo clínico previo determinar la relación de coste-efectividad incremental. 2) El segundo estudio evaluó la relación de coste-efectividad de usar o no la técnica de punción seca en miembro superior de pacientes con ictus en fase subaguda para disminuir la espasticidad. 3) El tercer estudio compara tres tratamientos para disminuir el dolor y mejorar la movilidad en la tendinopatía rotuliana usando ejercicios excéntricos y su combinación o no con electrólisis percutánea intratisular y punción seca. 4) El cuarto y último estudio valora la eficiencia de la realización o no de una sola sesión de punción seca en miembro superior de pacientes con ictus en fase crónica para la disminución de la espasticidad y mejora de la función. Resultados: Estudio 1: tanto la punción seca como la electrólisis percutánea intratisular fueron efectivas para el dolor plantar y de talón, aunque la relación de coste-efectividad fue favorable a la electrólisis percutánea a medida que pasan las semanas desde el tratamiento hasta el final del estudio. Estudio 2: el tratamiento con punción presentó mejor relación coste-efectividad considerando el porcentaje de respondedores según la escala MMAS, aunque no se obtuvieron datos concluyentes con los valores de calidad de vida. Estudio 3: en el tratamiento de la tendinopatía rotuliana la probabilidad de que el tratamiento combinado de electrólisis percutánea intratisular y ejercicios excéntricos fuera coste efectivo fue superior al uso de punción seca combinada con ejercicios excéntricos o al tratamiento sólo con ejercicios excéntricos. Estudio 4: una sola sesión de punción seca mejoró la calidad de vida a las 2 semanas del tratamiento y presenta una buena relación coste-efectividad frente a control. Conclusiones: las técnicas de punción seca y de electrólisis percutánea intratisular presentan buenas relaciones de coste-efectividad en el tratamiento del dolor plantar y de talón, en la tendinopatía rotuliana y en los tratamientos de la espasticidad de pacientes con ictus tanto en fase subaguda como crónica.
Descripción : Introduction: Dry needling is a minimally invasive technique that allows to treat different conditions of the neuromusculoskeletal system directly (e.g., muscle contractures, tendon injuries) and indirectly (e.g., chronic pain and spasticity). A solid, non-bevelled needle is used for this purpose, which causes a mechanical effect on the tissue and can be combined with other physical agents such as electrical currents, depending on the objectives of the treatment. Several studies have already evaluated its effectiveness in various pathologies; however, its economic impact has not yet been measured and its possible incorporation into daily clinical practice such as the rehabilitation of stroke patients could represent a great saving in costs compared to current alternatives. The aim of this thesis is to determine the cost of these techniques in different applications and to carry out different cost-effectiveness analyses to assess its efficiency in different pathologies. Material and methods: Four cost-effectiveness studies of these minimally invasive techniques have been completed in different areas: 1) The first study is a cost-effectiveness analysis comparing the dry needling technique with percutaneous electrolysis in plantar heel pain. The objective was to determine the cost of both techniques and from the data of a previous clinical trial to determine the incremental cost-effectiveness ratio. 2) The second study evaluated the cost-effectiveness of using or not using the dry needling technique on the upper limb of patients with subacute stroke to reduce spasticity. 3) The third study compared three treatments to reduce pain and improve mobility in patellar tendinopathy using eccentric exercises and their combination or not with percutaneous needle electrolysis and dry needling. 4) The fourth and last study assesses the efficiency of performing or not a single session of dry needling on the upper limb of patients with stroke in the chronic phase to reduce spasticity and improve function. Results: Study 1: both dry needling and percutaneous needle electrolysis were effective for plantar heel pain, although the cost-effectiveness ratio was in favour of percutaneous needle electrolysis from the beginning of treatment to the end of the study. Study 2: treatment with dry needling showed better cost-effectiveness considering the percentage of responders according to the MMAS scale, although no conclusive data were obtained with quality-of-life values. Study 3: in the treatment of patellar tendinopathy, the combined treatment of percutaneous needle electrolysis and eccentric exercises was more cost-effective than the use of dry needling combined with eccentric exercises or treatment with eccentric exercises alone. Study 4: a single dry needling session improved quality of life 2 weeks after treatment and was cost-effective compared to control. Conclusions: dry needling and percutaneous needle electrolysis techniques have good cost-effectiveness ratios in the treatment of plantar heel pain, patellar tendinopathy and in the treatment of spasticity in patients with stroke in both subacute and chronic phases.
